The short version

HIFU (high-intensity focused ultrasound) is a non-invasive energy-based treatment. No needles. Ultrasound waves converge at precise depths to heat tissue, triggering collagen remodelling over months. Most effective at the SMAS layer (4.5mm depth).

Thread lift is a minor surgical procedure. Absorbable threads (typically PDO — polydioxanone) are inserted under the skin via fine needles, providing immediate mechanical lift and triggering localised collagen production around each thread over months. The threads dissolve over 6 to 9 months while the collagen scaffold remains.

How each works

HIFU mechanism

Focused ultrasound passes through the skin without heating it and converges at a precise depth (1.5mm, 3.0mm, or 4.5mm depending on cartridge). At the focal point, tissue heats to 60-70°C, creating thousands of microscopic thermal coagulation points across the treatment area.

The body responds by depositing new collagen at these points over 3 to 6 months. Initial mild tightening may be visible within weeks; full effect at 3 months.

Thread lift mechanism

Under local anaesthesia, the doctor inserts barbed PDO threads through a fine introducer needle. The barbs grip subcutaneous tissue. The doctor uses the threads to physically pull tissue into the desired position, then trims the excess.

Mechanism is two-fold:

The threads gradually dissolve over 6 to 9 months. The collagen scaffold persists significantly longer.

For visible jowling or jawline blur

Thread lift +/- HIFU. The mechanical action of threads can re-define the jawline and reposition descending tissue immediately. HIFU can be added 4 to 6 weeks later for collagen reinforcement.

For midface flatness with mild laxity

Thread lift to reposition tissue, possibly combined with collagen-stimulating injections (Sculptra) for volume. HIFU alone is insufficient for true repositioning.

For patients who want no downtime

HIFU. The no-downtime profile means treatments fit around work and social calendar. Patients who can't have visible bruising during a wedding month, important presentations, or media appearances strongly prefer HIFU.

For patients with significant skin laxity

Honestly, surgical evaluation. Neither HIFU nor thread lift produces facelift-level results for advanced laxity. We'll say so during consultation if that's what you actually need.
